A Program of Options

Our benefits start the first of the month following 30 days of full time employment. You can select your benefit election on your first day of employment. Early enrollment is encouraged to allow vendors adequate time to set up your benefits and mail your cards out. In order to make a change mid-plan year you need to have a qualifying event (marital change, birth/adoption of child, loss/gained coverage, etc.). Documentation and enrollment is due in UKG within 30 days of the life event happening.

Resources to Help Navigate Enrollment

If you need help or have any questions, we are here to help. We have included some of the most commonly asked questions and key contacts to help you as you navigate enrolling in benefits.

You can self-guide your enrollment through UKG (click for how to guide) or call the benefit enrollment center at 888-598-2040 (Monday through Friday, 8 a.m. to 5 p.m.). Download the "Enrollment and FAQ" PDF for more resources.

You can only make changes during the year (outside of the annual Open Enrollment period) if you experience a qualifying event or family status change. These include:

  • A change in marital status
  • A change in number of dependents (birth or adoption)
  • A change in your eligibility status (i.e., full-time to part-time);
  • A change in your spouse’s/domestic partner’s benefits or employment status that results in a loss or gain of coverage; or
  • You become eligible for Medicare.
 

If you experience a qualifying life event, you must notify Human Resources within 30 days to be eligible to make changes to your benefits. If you fail to do so, you will be required to wait until the next annual Open Enrollment period to update your benefits.

You are able to make changes outside annual benefit enrollment if you have an IRS-qualified life event (QLE). Examples of this are a marital change, birth/adoption of child, spouse/dependent lost/gained coverage, or the death of a dependent within the last 30 days.

You will need to submit proper documentation showing proof of your QLE. Please ensure everything is submitted within 30 days of the life event. In order to submit we do need documentation uploaded into UKG, fax to 402-829-2963 or email MyHR@Immanuel.com, or view the "Enrollment and FAQ" PDF for more resources.

You may only change or cancel benefits during the year if you experience a qualifying life event (see above). Otherwise, you must wait until the Open Enrollment period to change your benefits elections, which will become effective the following Jan. 1.

If you recently left Immanuel, your employer-sponsored benefits will end, Please see this document for specifics!

Be sure the dependent is marked as a dependent. Also check the dependent has a SSN number in, their date of birth, their gender and be listed a qualified dependent (ie spouse, child, step-child, etc.).

  • If you select SimplePay Health,  you must complete the SimplePay Authorization Form.
    • Submit the proper dependent verification (if you added new dependents to plans). Instructions on how to upload are here.
    • Increasing coverage amounts or adding Supplemental Life Insurance outside the new hire benefit enrollment period requires the completion of an EOI form.  Amounts are subject to an EOI form. The benefit amount will be updated once approval has been received from the carrier.

  • You don’t pay providers or pharmacies at the time of visit. Instead SimplePay pays providers upfront and sends you a monthly statement.
    • The plan allows members to pay balances for 12 months without interest.
    • With SimplePay paying providers upfront and allowing members to make monthly payment.
