Immanuel Pathways is the Program of All-Inclusive Care for the Elderly (PACE®). It is a healthcare program designed specifically for older adults who need a nursing home level of care but want to stay aging at home. With PACE®, participants have a dedicated Care Team to coordinate everything, so they can live comfortably where they are. PACE® participants get the kind of care and therapies they need while living at home.

Empowering Independence

Serving adults 55+ in Nebraska and Iowa

PACE® provides comprehensive healthcare and social services to older adults (participants) still living independently in the community. Most of the participants who are in PACE® are dually eligible for both Medicare and Medicaid.

PACE® participants receive comprehensive, coordinated, customized care across various settings, including their homes and the Immanuel Pathways Center. PACE® is both a healthcare provider and a health plan.

An affordable model of care that puts you at the center.

Immanuel Pathways, our PACE® program, provides the entire continuum of medical care and long-term services and support required by older adults. These include primary and specialty medical care, such as audiology, dentistry, optometry, podiatry; home care; prescriptions; specialty care; respite care; transportation; adult day services, including meals, nutritional counseling, social work, personal care, and physical, occupational, and recreational therapies; as well as hospital and nursing home care, when necessary.

PACE® covers all Medicare Parts A, B, and D benefits, all Medicaid-covered benefits, and any other services or support that are medically necessary to maintain or improve the health and well-being of PACE® participants.

Medicaid-eligible individuals pay no premium for Immanuel Pathways services.

Serving Eastern Nebraska

Immanuel is the sole provider of PACE® in Nebraska and one of two providers in Iowa. We serve adults ages 55 and older in select zip codes of Douglas, Sarpy, Cass, Dodge, Saunders, and Washington Counties in eastern Nebraska.
