PACE®

Services offered at Immanuel Pathways of Central Iowa

PACE® care includes medical and personal care, rehabilitation, social interaction, medications, transportation, and more, all in one place so you or your loved one can live at home. Participants must receive all needed health care, including primary care and specialist physician services (other than emergency services), from Immanuel Pathways of Central Iowa or an entity authorized by Immanuel Pathways of Central Iowa. Immanuel Pathways of Central Iowa will be the sole service provider, and guarantees access to services, but not to a specific provider.

Quality care while living at home

We all want the best care for ourselves or our aging loved ones, but getting that care can be frustrating. If you have an aging loved one, you know what it's like to drive from appointment to appointment, manage all of their medications, and deal with doctors who don't talk to one another. You struggle with leaving your loved one at home, alone and without the supervision, company, and interaction they want and need. Our Immanuel Pathways centers centralize all of those services in one place.

  • On-call 24 hours per day, seven days per week

Transportation Services

  • Immanuel Pathways drivers ensure safe door-through-door travel between a participant's home and the day center, as well as other health care appointments.

Prescriptions & Over-the-Counter Medications

  • PACE® provides all medically necessary medications without co-payments
  • PACE® organizations offer Medicare Part D prescription drug coverage.
  • If you are in a PACE® program, you don't need to join a separate Medicare drug plan. If you do, you will lose your PACE® health and prescription drug benefits.
