Services offered at Immanuel Pathways of Southwest Iowa

PACE® care includes medical and personal care, rehabilitation, social interaction, medications, transportation, and more, all in one place so you or your loved one can live at home. Participants must receive all needed health care, including primary care and specialist physician services (other than emergency services), from Immanuel Pathways of Southwest Iowa or an entity authorized by Immanuel Pathways of Southwest Iowa. Immanuel Pathways of Southwest Iowa will be the sole service provider, and guarantees access to services, but not to a specific provider.

Your home, your care, your PACE®

Quality care while living at home

We all want the best care for ourselves or our aging loved ones, but getting that care can be frustrating. If you have an aging loved one, you know what it's like to drive from appointment to appointment, manage all of their medications, and deal with doctors who don't talk to one another. You struggle with leaving your loved one at home, alone and without the supervision, company, and interaction they want and need. Our Immanuel Pathways centers centralize all of those services in one place.
