Overview

What is a continuum of care?

Similar to a continuing care retirement community (CCRC), Immanuel’s continuum of care is a connected system of services that adapts as your needs change:

  • Independent living for active adults
  • Assisted living for daily support
  • Memory support for cognitive care
  • Skilled nursing and long-term care for complex health needs


Not every Immanuel community offers every care level, but Immanuel residents enjoy priority access to other Immanuel communities when care needs evolve. This means you can stay within a trusted organization that you are already familiar with and comfortable in.

For example, a resident who begins their retirement journey in independent living can transition to assisted living within their current community as their needs evolve. If the community they reside in does not offer assisted living, that resident will have priority access to other nearby Immanuel communities that offer the service level needed.

Independent living

Independent living at Immanuel is designed for healthy, active adults 55 and older who desire freedom without the burden of home upkeep. Our independent living communities feature:

  • Maintenance-free apartments, patio homes, or townhomes
  • Restaurant-style dining
  • Fitness and wellness programs
  • Social, educational, and spiritual activities
  • Pastoral care from certified and ordained clergy
  • Life enrichment programs like music, art, and travel


Independent living residents can enjoy independence, knowing they have priority access to higher levels of care if their needs change. That’s the reassurance of our continuum of care.

Immanuel’s 55-plus active living communities are specifically designed for retirees looking for hassle-free senior living. Our communities are ideal for retirees who want to trade their weekends mowing the lawn for Saturday mornings sipping coffee on their patio or balcony.

Living here is just a life of relaxation and great caring. We don’t worry about lawns, snow, or maintenance. If you have something that needs to be fixed in your apartment? Call maintenance. There are so many wonderful people here.

Assisted living

Priority access & support when you need it

Assisted living communities are for seniors whose health or well-being requires a higher level of support. This is determined through a community health assessment in accordance with state regulations.

Immanuel’s assisted living options offer comprehensive care. This includes a spectrum of services tailored to meet the individual needs of you or your loved one. This provides the perfect balance between independence and the support necessary to thrive.

Assisted living provides help with daily tasks while preserving independence. Services include:

  • Transportation and housekeeping
  • Social and wellness programs
  • Restaurant-style dining
  • Personalized care plans


Residents of Immanuel who move from independent living to assisted living receive priority access, ensuring a seamless transition and continuity of personalized care.

Memory support

Skilled nursing & long-term care

Specialized support for complex needs

Skilled nursing provides specialized medical care from professionals, including nurses and therapists. Older adults with complex needs receive 24-hour supervision, treatment, and rehabilitation. Long-term care, including at-home or facility-based options, supports daily activities. Services include:

As part of Immanuel’s continuum of care, a resident can begin their retirement journey in independent living. As the resident’s health needs change, they can transition to assisted living. When the resident needs more advanced care, Immanuel has skilled nursing and long-term care communities. Similar to assisted living and memory care, Immanuel residents have priority access to these advanced care communities.
