Immanuel Pathways Southwest Iowa

Serving seniors in southwest Iowa, Immanuel Pathways offers PACE® (Program of All-inclusive Care for the Elderly) to seniors, enabling nursing home qualified individuals to remain happy and healthy in their homes. Participants receive transportation to our Immanuel Pathways centers, where they receive medical care, professional support and social opportunities. 

At the heart of this intuitive program is its compassionate interdisciplinary team of doctors, therapists and other professionals who provide an array of services including coordinated senior care, transportation, prescriptions, in-home care, equipment, therapeutic day social activities and more. Best of all—Medicaid eligible individuals pay no premium for Immanuel Pathways services. 

The interdisciplinary care team means all care providers are on the same page and informed, lifting a weight off the shoulders of family members who no longer have to worry about coordinating services.

PACE® participants may be fully and personally liable for the costs of unauthorized or out-of-PACE® program agreement services. Emergency services are covered. Participants can disenroll at anytime.
